In requesting a tutor, you must be willing to put in the hard work. Filling out this form means you understand this and are committed to:

1. Meeting with your tutor weekly.
2. Communicating in a timely fashion.
3. Showing up to your tutoring sessions on time (5 minutes early is even better).
